Jacksonville - Chosen Hammonds, four months of Jacksonville, entered into eternal rest on Saturday, April 16 2022 at his residence.
Celebration of Life service will be held 12pm Tuesday, April 26, 2022 at River of Life Church in accordance with the Covid-19 mandate.
He is survived by his parents, siblings, and a host of relatives and friends.
